Point-In-Time Location Preference
We will do our best to match you with your first preference, but may need to match you with an alternative neighborhood. You will receive an email confirmation with the final placement.

Note: if you select "Yes" we will assign you a driving map, based on your neighborhood preferences. You will be responsible for driving yourself and your team members throughout the duration of the PIT Count.

PLEASE READ THE INFORMATION BELOW CAREFULLY

I certify that I am an adult, 18 years of age or older, and I am offering my services to the 2026 District of Columbia Point-in-Time Count (2026 PIT Count) and The Community Partnership for the Prevention of Homelessness (TCP) or one of its coordinating entities on a strictly volunteer basis. I understand and acknowledge as a volunteer that the COVID19 virus is still prevalent in the United States, including the District of Columbia, and that during the 2026 PIT Count agree to take reasonable precautions as highlighted below. 

 

understand that the volunteer Health and Safety Guidelines below will apply to me during the 2026 PIT Count. As a volunteer I affirm the following:  

 

  • I have been fully vaccinated from COVID-19 virus (14+ days post-completion of the primary series of FDA authorized COVID-19 vaccine) or I have taken or will take precautions as outlined by the CDC; 

  • I understand that wearing a face mask or covering and other PPE will limit my exposure to COVID-19 and other airborne diseases to include influenza and pneumonia and that wearing a face covering is strongly encouraged; 
  • must at all times social distance from other volunteers and individuals who are the subject of the 2026 PIT Count, meaning, to the extent feasible, remaining at least six feet apart; 
  • I will sanitize my hands frequently during the 2026 PIT Count; and
  •    
  • I will follow the safety and health protocols that have been (or may be) implemented for the 2026 PIT Count.  

 

will not report to the 2026 PIT Count (January 282026), under the following circumstances: 

 

  • have been exposed to a person with a confirmed or suspected case of COVID-19 or another airborne disease, including influenza or pneumonia; on or after January 10, 2026; 
  • was diagnosed with COVID-19 or another airborne disease, including influenza or pneumonia on or after January 10, 2026;  
  •  I have a fever, am coughing, have chills, shortness of breath or have other flu-like symptoms;  
  • If I cannot follow the safety and health protocols that have been implemented for the 2026 PIT Count. 

 

If I cannot volunteer on January 28, 20626 for any of the foregoing reasons, I will notify The Community Partnership by email at pit@community-partnership.org.

 

   I acknowledge and understand that I voluntarily applied to serve as a surveyor for the 2026 PIT Count and as a volunteer I will not be paid for my volunteer services. I acknowledge, as a volunteer, I am not eligible for any benefits traditionally associated with employment situations, including workers’ compensation benefits or health insurance.  If I am injured or become ill as a result of my volunteer service, I am responsible for my own health care coverage and any fees associated with the request for or receipt of any associated medical care, or any other associated fees, costs, expenses, or claimsI also understand that I am not eligible for unemployment compensation benefits when my volunteer assignment ends.  

 

I understand that I will be conducting the 2026 PIT Count surveys using my personal mobile device (or a company-owned mobile device that I have permission to use for this purpose or a TCP issued device) for which I will not be compensated. I further understand that I will be required to download an application (app), ArcGIS Survey123, for the purpose of conducting the 2026 PIT Count surveys. If during the course of using my personal (or company/ owned) mobile device, it is damaged, stolen, destroyed, or otherwise made inoperable, I understand that I will not be compensated by the District of Columbia, TCP, or any of the coordinating entities for any cost associated with the loss of my mobile device, the loss of use of my mobile device, or any other incidents, including misuse of the device, associated with the loss of my mobile device.   

 

If I am issued a TCP device, I agree to use the device with reasonable care and avoid causing any damage to the device and agree to use the device only for purposes associated with the 2026 PIT Count. The device must be returned in the same condition as when it was received. 

 

I understand that while conducting the 2026 PIT Count surveys, I will be collecting personally-identifying information (names/birthdates) and personal health information from the individuals with whom I will be interacting and I will be storing that information on my personal mobile device (or other issued device) until the information is successfully submitted in the app. I understand that the personally-identifying and/or health information does not belong to me and that I may not use (or misuse) or transfer (or otherwise share (orally or in writing) the information to anyone, except as required by law or to the designated 2026 PIT Count lead, for any purpose. I understand that dissemination or misuse of the personally-identifying or health information collected as a part of the 2026 PIT Count could subject me to civil or criminal penalties or referral to law enforcement for appropriate action(s).   


I understand and agree that , if instructed, I will: 

    • Add the Survey123 application (app) to my phone;  
    • Use the Survey123 app to collect information from designated individuals; 
    •  “Send” all surveys collected on the Survey123 app 
    • Empty the “sent” survey folder of all information; and 
    • At the end of my volunteer assignment, delete the Survey123 app from my phone. 


I understand that my volunteer activities associated with the 2026 PIT Count may expose me to people with infectious diseases. I fully understand, appreciate, and assume the risks that are inherent in these volunteer activities.  I hereby assume the risk of bodily injury, illness, death, medical treatment, and/or property damage that may result from my volunteer surveyor activities, even if such result from the negligence of the District or TCP or its officers, directors, employees, or agents, or any of the coordinating entities.  

 

I hereby release, discharge, and agree to indemnify and hold the District, TCP, and/or any of the coordinating entities harmless from, and waive on behalf of myself and my heirs and personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and/or compensation for damage or loss to myself and/or my property that may be caused by any act, or failure to act by the District, TCP, and/or any of the coordinating entities, or that may otherwise arise in any way in connection with the 2026 PIT Count. 

 

I understand that this release discharges the District, TCP, and/or any of the coordinating entities from any liability or claim that I or my heirs, personal representatives, or minors for whom I am responsible may have against the District or TCP with respect to bodily injury, illness, death, medical treatment, and/or property damage that may arise from or in connection with my volunteer activities.  This liability waiver, assumption of the risk, and release extends to the District and TCP together with all of its officers, directors, employees, or agents.    

 

I also understand that during my volunteer assignment, I might be the subject of a photograph(s) or might be in the background of a photograph(s). I hereby grant permission to TCPto use photographs and/or video of me taken on January 28th and 29th2026,during the 2026 PIT Count in publications, news releases, online, and/or in other communications related to documenting the 2026 PIT Count.  I understand that there will be no payment associated with this release and waiver and that I am not entitled to any payment.   

 

I agree that this release will be governed by the laws of the District of Columbia and the exclusive jurisdiction for any dispute arising from this release will be in the Superior Court of the District of Columbia. 

 

I CERTIFY THAT I HAVE READ THIS ASSUMPTION OF RISK AND WAIVER OF LIABILITY FORM AND THAT I UNDERSTAND THE CONTENT AND THAT I ACCEPT AND WILL ABIDE BY THE TERMS SET FORTH HEREIN.
